How to sign up for the patient portal
Give us a call with your email address and we will send you a link to sign up. All you have to do is follow the instructions provided in the link.
Filling out new patient paperwork on the portal
You can request to have paperwork sent to your portal to be filled out prior to the appointment.
Where to find the new patient paperwork
Click on the DOCUMENTS tab on the side.
Click on MY DOCUMENTS to reveal the other options
Click on NEW INTAKE FORMS
Fill out the forms
